Improving Care Transitions Symposium

Patients are often most fragile at the time of hospital discharge. This symposium explores research experiences and needs in this important area, with findings shared on why pharmacists in a team approach have such a vital role to play during patient care transitions. Speakers will report on physician and pharmacist interest in collaboration, and will describe strategies in use to communicate across hospital and community pharmacy systems, and patients.
